Filing for California State Licensure

1. Call the Speech Pathology and Audiology Examining Committee for an application at (916) 920-6388, or download the application from http://www.slpab.ca.gov.

2. Make sure all of your signed ASHA hours are in your Clinic file. A copy of your clinic records and licensure application will be kept in your file at SDSU.
